This is the Porchia, Ridley & Kinney Foundation Assistance, Referral Outreach Initiative Application for Assistance. This application is offered to improve the overall Wellness of the Community. PRK Foundation has a planning process that brings people together from different parts of the whole family’s life. PRK Foundation is bringing together resources from our community partners in healthcare, mental health, education, housing, labor, justice, disaster, immigration, transportation, agriculture, religious/faith based and the environment to work together to coordinate services and assist and support the family goals and needs. Cost: There is no cost to receive assistance.
